What to do, in order
Check that the crypto really left: find the transaction ID in the sender's withdrawal history; it should show as confirmed/success on the block explorer. Binance says to contact the sending platform if it's unconfirmed or failed
Binance, missing or wrong memo: log in and open Tag/Memo Recovery, choose the coin, enter the exact amount (up to 8 decimal places, no separators) and the TxID, then click Submit and confirm
Binance, unsupported coin or network: open self-service recovery, paste the TxID and click Submit, check the coin, network, deposit address and amount, then click Confirm. You may be asked for more, such as a video
OKX, missing or wrong tag: use Get help with tag/memo issues in the OKX Support Center
OKX, unsupported coin: withdraw it to a platform or wallet that supports it, using the same network as their deposit address

FAQ
Does Binance charge for recovery?
Binance says you're only charged if recovery succeeds, and you can see the fee under Application Records. For a missing memo, the fee is the coin's network withdrawal fee; if what's left is below the minimum withdrawal amount, it can't be recovered.
Where does the recovered crypto go?
Binance says a missing or wrong memo deposit is returned to the sender's address. If it came from another platform, contact that platform to have it credited.
What if the wrong memo belongs to someone else?
Binance says the deposit may already be in that user's account, and it can't help in that case.
Which coins need a memo?
Binance's examples include BNB, XLM, XRP and EOS. If the deposit page shows a tag or memo, enter it. For more, see What are a memo and a tag?.
Someone says they can recover it if I pay first. Should I?
No. Asking for money up front to recover funds is a common warning sign. Recovery only goes through the exchange's own website or app.
